H1N1 vaccine available for children

ELMENDORF AIR FORCE BASE, Alaska --  The 3rd Medical Group staff will provide the H1N1 and seasonal flu vaccine for children, 6 months old and up, who attend the Denali, Sitka or Ketchikan child development centers 4-6 p.m. Oct. 21 at  the Denali Child Development Center and 4-6 p.m. Oct. 22 at the Ketchikan School Age Program.

Parents must download, fill out and sign the consent form at www.hss.state.ak.us/dph/DPHPP/pandemicflu/PDFs/izkit/2-F-iii_LAIV.doc. The consent form will also be provided on site during the immunizations.
